Below you can find and download all documents and forms that are necessary for admission process in Little Europe Daycare. If you have more than one child we will need forms for each child except for the contract.

• The Contract
• State of Illinois Certificate of Child Health Examination (DCFS)
• Aplication / Record of Child Information (DCFS)
• Consents to Daycare Provider (DCFS)
• Late Pick Up Police
• Evacuation in an Emergency Transportation Waiver

We will also need copies of your child/ren's Birth Certificate and Medical Insurance
